What Is Muscle Atrophy?

Muscle atrophy is the loss of muscle tissue. It happens when muscle fibers shrink or break down faster than they're being rebuilt. This can occur gradually over months or years, or relatively quickly following injury, illness, or prolonged inactivity.

Not all muscle loss is the same. There are two main types:

Disuse atrophy — the most common type, caused by physical inactivity. Muscles that aren't regularly used shrink. This happens to people with sedentary jobs, those recovering from injury, and older adults who become less active over time.

Neurogenic atrophy — caused by nerve damage or disease. When the nerve signals that tell a muscle to contract are disrupted, the muscle deteriorates. This is more serious and harder to reverse.

Most people experiencing unexplained muscle loss are dealing with disuse atrophy or age-related muscle loss (sarcopenia), rather than neurogenic causes.

Common Causes of Muscle Loss

1. Physical Inactivity

The most straightforward cause. Muscles respond to demand — if you're not placing regular stress on them through movement or resistance, they shrink. Even a few weeks of significantly reduced activity can produce noticeable muscle loss, particularly in the legs.

2. Aging (Sarcopenia)

After age 30, adults lose roughly 3–5% of muscle mass per decade. After 60, the rate accelerates. This age-related muscle loss — called sarcopenia — is normal but not inevitable. It's significantly influenced by activity level, diet, and hormonal changes.

3. Inadequate Protein Intake

Muscle tissue is built from protein. If you're not consuming enough, your body can't repair and rebuild muscle fibers after the normal wear of daily activity. Most adults need between 0.7–1 gram of protein per pound of body weight daily to maintain and build muscle, though this varies by age and activity level.

4. Caloric Deficit

Sustained caloric restriction — especially from crash dieting — forces the body to break down muscle tissue for energy when fat stores aren't sufficient. This is why rapid weight loss often includes a significant loss of lean muscle mass alongside fat.

5. Hormonal Changes

Testosterone and estrogen both play roles in maintaining muscle mass. Declining levels — due to aging, menopause, or certain medical conditions — can accelerate muscle loss. Low levels of growth hormone and IGF-1 (insulin-like growth factor) also contribute.

6. Chronic Illness

Several conditions directly cause or accelerate muscle atrophy:

  • Cancer and cancer cachexia
  • Heart failure
  • Kidney disease
  • Diabetes (uncontrolled)
  • HIV/AIDS
  • COPD
  • Prolonged infections

7. Immobilization or Bed Rest

Following surgery, fracture, or serious illness, patients are often immobilized for extended periods. Muscle loss during this time can be rapid — research shows that complete bed rest can cause healthy adults to lose up to 1–1.5% of muscle mass per day in the first week.

8. Corticosteroid Use

Long-term use of corticosteroid medications (like prednisone) is a well-known cause of steroid-induced myopathy — a form of muscle weakness and atrophy that affects primarily the proximal muscles (hips, thighs, shoulders).

Signs You May Be Losing Muscle Mass

Muscle loss isn't always obvious at first. Watch for:

  • Visible decrease in muscle size — particularly in the arms, thighs, or calves
  • Weakness in daily tasks — difficulty rising from chairs, climbing stairs, or carrying groceries that used to feel easy
  • Fatigue during low-intensity activity — feeling worn out from tasks that previously required no effort
  • Slower metabolism and weight gain — muscle burns more calories at rest than fat; losing it slows your metabolic rate
  • Poor balance or coordination — reduced muscle strength contributes to instability
  • Longer recovery from exercise — muscle that has atrophied recovers more slowly from physical stress

If muscle loss is rapid, asymmetric (affecting one side of the body), or accompanied by pain, numbness, or neurological symptoms, see a doctor. These can signal nerve damage or underlying disease.

How to Stop and Reverse Muscle Atrophy

Resistance Training

The most effective intervention for both preventing and reversing muscle atrophy is progressive resistance training — exercises that challenge muscles against load. This includes weightlifting, bodyweight exercises, resistance bands, and machine-based training.

For older adults or those recovering from atrophy, even light resistance training produces measurable gains. The key is consistency and progressive overload — gradually increasing the challenge over time.

Aim for at least 2–3 sessions per week targeting all major muscle groups.

Prioritize Protein

Increase protein intake, particularly around exercise. Research consistently shows that consuming 20–40 grams of high-quality protein within a few hours of resistance training maximizes muscle protein synthesis.

Good sources: eggs, lean meats, fish, Greek yogurt, cottage cheese, legumes, and protein supplements if needed.

Older adults may need more protein than younger people to achieve the same muscle-building response — some research suggests 1.2–1.6 grams per kilogram of body weight per day for those over 65.

Stay Active Throughout the Day

Formal exercise matters, but so does overall movement. Extended sitting is independently associated with muscle loss. Breaking up sedentary time with short walks, standing, and light activity helps maintain muscle tone between workouts.

Address Underlying Causes

If muscle loss is related to a medical condition, hormonal imbalance, or medication, treating the underlying cause is essential. Work with your doctor to:

  • Optimize hormone levels if deficient
  • Adjust medications causing myopathy
  • Treat conditions contributing to cachexia or wasting
  • Rule out neurological causes if atrophy is asymmetric or rapidly progressing

Nutrition Beyond Protein

Other nutrients that support muscle health:

  • Creatine — well-supported evidence for improving strength and muscle mass, especially in older adults
  • Vitamin D — deficiency is linked to muscle weakness; supplementation helps in those who are deficient
  • Omega-3 fatty acids — may reduce muscle protein breakdown, particularly in aging populations
  • Leucine — an amino acid that directly triggers muscle protein synthesis; found in whey protein, eggs, and meat

When to See a Doctor

Consult a physician if:

  • Muscle loss is rapid or unexplained
  • One limb is noticeably more affected than others
  • You have accompanying symptoms like pain, tingling, or numbness
  • Weakness is affecting your ability to function safely
  • You have a known condition associated with muscle wasting

A doctor may order blood tests (to check protein levels, hormones, inflammatory markers), imaging, nerve conduction studies, or refer you to a specialist depending on findings.

Losing muscle mass is common — especially with age, inactivity, or inadequate nutrition — but it's not something you simply have to accept. The combination of regular resistance training, adequate protein, and addressing any underlying medical causes is effective for most people. The earlier you address it, the easier it is to reverse. Waiting makes recovery longer and harder.
