Prevent Hangovers: A Doctor's Simple Trick to Avoid the Aftermath of Drinking Alcohol
Hangovers are not mysterious. The science of why they happen — and what actually reduces them — is fairly well understood, even though the popular mythology around them is dense with false remedies and half-truths. Understanding what a hangover actually is helps cut through the noise and focus on the interventions that have genuine evidence behind them.
This article covers the physiology of hangovers, what doctors and researchers actually recommend for prevention, and an honest assessment of which popular "tricks" work and which don't.
What a Hangover Actually Is
A hangover is not a single thing — it's a cluster of symptoms produced by several overlapping biological processes that occur when you consume alcohol and during the subsequent recovery period. Understanding these processes reveals which prevention strategies are grounded in something real.
Dehydration and electrolyte loss are the most famous contributors. Alcohol is an antidiuretic hormone (ADH) antagonist — it suppresses ADH, which is the hormone that tells your kidneys to reabsorb water. When ADH is suppressed, your kidneys excrete far more water than usual. You lose electrolytes along with that fluid — sodium, potassium, and magnesium especially. The classic hangover symptoms of thirst, headache, fatigue, and dizziness are largely driven by this mechanism.
Acetaldehyde accumulation is another major contributor. When your liver metabolizes alcohol (ethanol), the first metabolite is acetaldehyde — a toxic compound that causes flushing, nausea, increased heart rate, and inflammation before it is further converted to acetate and then to carbon dioxide and water. The worse your liver is at clearing acetaldehyde (influenced by genetics, liver health, and how much you drank), the more it accumulates and the worse you feel.
Inflammation is the third major mechanism. Alcohol consumption elevates inflammatory cytokines — signaling molecules involved in immune response — and the inflammatory state they produce contributes significantly to the cognitive symptoms of hangovers: the brain fog, difficulty concentrating, and mood disruption. This is why hangovers often feel somewhat like mild flu symptoms.
Sleep disruption compounds everything. Alcohol disrupts sleep architecture — it suppresses REM sleep and causes earlier waking, which means even if you sleep eight hours after drinking, the sleep quality is poor. This alone causes many of the next-day symptoms attributed specifically to alcohol. Restorative sleep is one of the most fundamental pillars of health, and alcohol-disrupted sleep undermines recovery across every body system simultaneously.
The Doctor's Actual Recommendation: Less Alcohol
The most evidence-backed hangover prevention strategy is straightforward and unsatisfying: drink less. No supplement, food, or pre-drinking ritual can fully compensate for consuming large amounts of alcohol. The severity of hangovers correlates dose-dependently with alcohol intake — the more you drink, the worse the hangover, with no exception for preparation methods, genetic factors, or fitness level that eliminates this relationship.
What doctors actually recommend, when asked, is pacing — one standard drink per hour, which is approximately the liver's maximum processing rate for alcohol. Staying at or under this pace prevents acetaldehyde accumulation because the liver can keep up. Above this rate, acetaldehyde builds faster than it's cleared. This is the primary mechanism by which drinking speed, independent of total alcohol consumed, affects how you feel the next day.
Alternating alcoholic and non-alcoholic drinks reduces total consumption while maintaining social participation, and is the most practical pacing strategy for settings where a specific drinking rate is hard to maintain. Choosing lower-sugar mixers and drinks reduces both the sugar load and congener content that contribute to hangover severity.
Hydration: Essential, But Not the Whole Story
Drinking water is genuinely helpful, but the evidence suggests it attenuates rather than prevents hangover symptoms when alcohol consumption is high. The correct hydration strategy is threefold: drink water before you start drinking (pre-hydrating), maintain water intake during drinking (at least one glass of water per alcoholic drink), and drink a large glass of water before bed.
Water alone doesn't replace the electrolytes lost through alcohol-driven diuresis. Adding electrolytes — particularly sodium and potassium — to the pre-bed hydration is more effective than water alone. Oral rehydration solutions, sports drinks, or simply adding a pinch of salt to water with a potassium source improves electrolyte replenishment. Maintaining proper hydration and electrolyte balance supports blood sugar regulation, which is also disrupted by alcohol and contributes to morning weakness and shakiness.
Food: Before, Not After
Eating before drinking is genuinely effective — but the mechanism is not "soaking up alcohol." Food slows gastric emptying, which slows the rate at which alcohol enters the small intestine and is absorbed into the bloodstream. This produces a lower peak blood alcohol concentration for the same amount consumed and gives the liver more time to process each unit of alcohol before the next arrives.
The composition of the food matters. Fat and protein are more effective at slowing gastric emptying than carbohydrates. A meal containing protein and fat before drinking produces more meaningful reduction in alcohol absorption rate than a high-carbohydrate meal. The old recommendation to eat "bread and pasta" before drinking is partially right (food helps) but wrong on composition (fat and protein are better).
Eating during drinking also helps through the same mechanism. Eating after drinking does not meaningfully affect alcohol already in the bloodstream but does support blood sugar and may reduce morning symptoms through that mechanism. The gut health effects of alcohol are also significant — alcohol disrupts the gut microbiome and intestinal permeability, and eating whole foods before drinking rather than only after supports better gut recovery.
Congeners: Why Drink Type Matters
Congeners are chemical byproducts of fermentation and aging — compounds other than ethanol present in varying amounts in different alcoholic beverages. Dark spirits (bourbon, whiskey, brandy, dark rum) contain significantly higher congener loads than lighter spirits (vodka, gin) and most wines.
Multiple studies have found that beverages with higher congener content produce more severe hangovers at equivalent alcohol doses. Bourbon produces significantly worse hangovers than vodka at the same blood alcohol level. The mechanism involves congeners adding their own direct toxicity and possibly interfering with acetaldehyde metabolism.
If you are choosing between a bourbon and a vodka drink with equal alcohol content, the vodka will likely produce a milder next-day effect. This does not mean dark spirits are "worse" in some general sense, only that at equivalent doses they tend to produce more severe hangover symptoms.
Supplements With Actual Evidence
Several supplements have been studied for hangover prevention with varying levels of evidence.
N-acetyl cysteine (NAC) supports glutathione synthesis. Glutathione is the body's primary antioxidant and is central to acetaldehyde metabolism. Taking NAC before drinking may support the liver's capacity to process acetaldehyde more efficiently. Standard doses used in studies are 200-600mg taken before drinking begins.
B vitamins — particularly B1 (thiamine), B6, and B12 — are depleted by alcohol metabolism and support neurological function and energy metabolism. Alcohol specifically reduces thiamine absorption, and thiamine deficiency contributes to the neurological symptoms of hangovers. A B-complex supplement before bed is low-risk and potentially helpful.
Prickly pear (Opuntia ficus-indica) extract showed promising results in a randomized controlled trial published in the Archives of Internal Medicine, which found that taking prickly pear extract five hours before drinking significantly reduced hangover severity, particularly nausea, dry mouth, and anorexia. This is one of the few hangover prevention supplements with a proper RCT behind it.
Dihydromyricetin (DHM), a flavonoid found in Japanese raisin tree extract, has been studied for its effects on alcohol metabolism and shows interesting results in animal studies. Human evidence is much more limited. Supporting the body's natural antioxidant systems through nutrition is more reliably beneficial than most single supplements.
What Doesn't Work
Coffee the next morning addresses the fatigue symptom but does not treat the underlying hangover and can worsen dehydration. It provides symptom management, not cure.
"Hair of the dog" — having another drink to treat the hangover — genuinely reduces some symptoms temporarily, primarily because it raises blood alcohol levels and blunts the withdrawal-like component of hangover. But it delays rather than prevents the full hangover, and for people who drink frequently, it creates risk of escalating toward dependence.
Activated charcoal does not bind ethanol effectively. Most commercial hangover remedies are combinations of vitamins, electrolytes, and herbal ingredients in variable doses with limited rigorous trial evidence.
The Hangover Prevention Summary
The interventions with the best evidence, in rough order of effectiveness: drink less (primarily less total alcohol and slower pacing), eat fat and protein before drinking, maintain hydration with electrolytes before and during drinking, choose lower-congener beverages, take a B-complex and consider NAC or prickly pear extract before drinking, ensure you get the best possible sleep, and rehydrate with electrolytes before sleep and upon waking.
None of these eliminate the physiological consequences of alcohol consumption at high doses — they attenuate them. For people who drink socially and occasionally experience significant hangovers, these strategies genuinely make a meaningful difference. For people who drink heavily regularly, the better conversation is about the broader health implications — because at that level of consumption, building a truly healthy lifestyle becomes the more important goal.
